What is covered? * School physicals * Sports physicals, if needed (Participation forms must be provided by the parent or guardian) Does my child qualify? * Your child must be 18 years old or younger; be uninsured and have no other insurance options * Parent or legal guardian must be present for a child to be seen by a physician and have any physical or immunization preformed. How do I sign up? * A parent or legal guardian must secure a "ticket" for each child through this Eventbrite registration. Once complete, the parent or legal guardian will receive an email through Docusign to complete the required registration paperwork. * Families with more than one child, need to reserve an individual ticket for each child. For example: a family with three (3) children would secure three (3) tickets back to back, such as the 8:15AM, 8:30AM and 8:45AM tickets.
